JCOD CARE MANAGEMENT (JCM)

JCM (JCOD Care Management) is an intensive, person-centered care management program for justice-involved individuals. Every eligible participant receives a dedicated care manager to help coordinate health, social, and other needs. Program goals include: reducing recidivism, improving management of behavioral and physical health conditions, and supporting successful reentry and continuity of care.

You Deserve a Care Manager Who Understands Your Needs

Our JCM program is led by expertly trained care managers with lived experiences who understand the challenges and obstacles you may be facing. These are individuals who have been in your shoes and know the complexities of your situation. In plain language, they can speak your language.

After enrolling in the program, you will meet with your care manager regularly to create and follow a clear road map for your overall wellness. Click below to get started on your journey towards successful reentry.

Program Details

 

Eligible participants receive care for a 6-12 month period (with a possible 6-month extension) where they will work with Care Managers to establish goals and obtain the support needed to reach those goals. Participants can receive the following services:​

Eligibility Requirements

 

  • Ages 18+

  • Current LA County resident or transitioning to LA County following incarceration

  • History of justice system involvement

  • Self-disclosed health need (no formal diagnosis required)
